Why these checks: what owners reported

NHTSA complaints for the 2022 Mazda 5, grouped by the part owners named. Open a row to read what goes wrong.

  • Exterior lights14No crashes

Headlights develop a loud whine, hum, or grinding noise. It can continue for a few minutes after the lights shut off. Dealers trace it to the cooling fan motor inside the LED headlight assembly, and owners say debris from manufacturing wears out the fan bearings. A failing fan can let the lamp overheat and burn out. The noise usually starts between about 29,000 and 41,540 miles, often just after the warranty ends. The fix is a full headlight assembly, which owners say costs $2,500 or more. A few also report dim or uneven beams.

  • Airbags132 crashes

"Air Bag System Malfunction" warnings show up on the dash and in the phone app. They often start in the first few thousand miles, sometimes right after a seat adjustment. Owners cite a service bulletin that points to the wiring harness for the side airbag in the seat. Many need repeat dealer visits. Other reports describe the passenger airbag-off light staying on with an adult in the seat and curtain airbags going off without a crash. In two crashes that caused injuries, the airbags did not deploy.

  • Electrical101 crash

Batteries go dead after the car sits for a few days. Some owners now carry a jump pack after several battery replacements. Dealers blame a constant power draw or a faulty battery. Other reports describe critical malfunction alerts, internal network faults, and sudden loss of power on the highway with the check engine and battery lights on. Two complaints involve fire, including a heated windshield element that smoked and cracked the glass. Repairs to airbag wiring under the seat are also logged here, with owners waiting weeks for parts.

  • Transmission9No crashes

Transmissions fail in several ways. Automatics refuse to move in gear or rev above 5,000 RPM, and the failures come back after valve body replacements and even a full transmission swap. Manual gearboxes grind on downshifts as the synchronizers wear, with grinding reported around 20,000 miles. Other complaints cover a failed transfer case on an almost-new car, a cracked cylinder head spraying oil, a leaking timing chain cover gasket, and a rear whine at highway speed. Most of these failures come with no warning light.

  • Engine7No crashes

Cold starts bring a sharp metallic ticking that owners link to the hydraulic valve lash adjusters. A service bulletin covered this problem on earlier model years. Some engines burn enough oil between changes to trigger a low-oil warning, and dealers find no leak. More serious reports describe a cracked cylinder head that leads to overheating, smoke from the engine bay, and a pull-over-immediately alert. Owners also report a coolant control valve stuck open and a warranty extension notice for a thermostat that can fail in the open position.

  • Automatic emergency braking52 crashes

Automatic emergency braking fires with nothing ahead and brings the car to a full stop. Owners report it on highways, in intersections, and in slow traffic. The alert often sounds at the same moment the brakes slam on, so the driver gets no advance warning. Dealers often cannot reproduce the fault. Two complaints involve crashes. In one, a false stop led to the car being hit from behind. In the other, the system gave no warning and added no braking before impact.

  • Acceleration and cruise control3No crashes

Engine RPM surges while the car is stopped or slowing down, and the car lurches forward against the brake. One report blames this on a large shoe pressing both pedals at once. A separate complaint describes a transmission control unit fault with code P061B that five repair attempts failed to fix. The main risk is the car moving unexpectedly near an intersection or over a curb. No crashes or injuries are recorded in this category.

  • Body and structure3No crashes

Rattles from behind the glove box and the head-up display start as early as 2,000 miles. Some cars make a loud, high-pitched reedy noise at highway speed in strong wind. It drowns out music and calls, and Mazda has not addressed it. In one report, a sunroof shattered outward at highway speed with nothing hitting it, throwing glass at the people inside and toward traffic behind. The interior noises are an annoyance. The sunroof failure is an injury risk.

In owners' own words

Exterior lights
Over the past several weeks, we began hearing a pronounced noise coming from the front of the vehicle, which the dealership diagnosed as a failing motor within the driver-side headlight unit. Given that this is a critical safety component, and part of the upgraded package purchased when the vehicle was new — we were surprised to encounter this issue on a vehicle of 4 years and 41,540 miles. AirbagsCrash reported
I was rear ended while at complete stop by someone going over 40mph on 12/19/22. My airbags did not deploy and my seat belts did not lock out. Directly after being hit my phone flashed a safety fracture malfunction on the dash board and then sent the notification to the mazda app on my phone.

Electrical
The vehicle unexpectedly caught fire and the results could have been catastrophic. Almost new car under warranty. All details are in the uploaded PDF letter and photos. I have contacted Mazda and they claim they do not investigate, only insurance company will investigate. My insurance company will pay me for the total loss but does not plan to investigate. I think this is dangerous and unconscionable. Thank you for any help or guidance.

Transmission
with less than 7000 miles on a new Mazda3, the transfer and transfer housing went out. This meant the all wheel drive did not operate properly, which could have caused an accident in the right conditions. This was covered by warranty, as well as a shock, strut, and numerous related parts. In total, the warranty covered the total $4395 to repair a car with less than 7000 miles on it.

Filed with NHTSA, April 2023

Transmission
The timing chain cover gasket failed and is leaking oil at 66,000 miles. It is currently being addressed at Borgman Mazda in Grand Rapids. No crash or safety risk. There were no warning lamps, it was found by an separate inspection not from the dealership.

Recalls to confirm (1)

Recall repairs are free at any Mazda dealer, but the seller may never have had them done. Check each campaign number against the VIN.

Mazda North American Operations (Mazda) is recalling certain 2016-2023 MX-5 Miata vehicles. A software error in the air bag control module may cause the front air bags to deploy with excessive force during a crash. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection.".

Risk: An air bag that deploys with excessive force can increase the risk of injury during a crash.

Fix: Dealers will update the Sophisticated Air Bag Sensor (SAS) control module software, free of charge. Owner notification letters were mailed November 7, 2024. Owners may contact Mazda customer service at 1-800-222-5500. Mazda's number for this recall is 6924I.
